1 Handling of Personal Information

4. Outsourcing of Personal Information

For the purpose of providing better services in our business operations, we outsource part of our work to external parties within the scope necessary to achieve our objectives. This includes tasks such as mailing guidebooks and newsletters, accepting and processing payments for products and services, and system operation and management.

When outsourcing business that handles personal information, we will select an outsourced company that is recognized as handling personal information appropriately, and we will stipulate necessary matters for preventing leakage of customer personal information through appropriate management and confidentiality in contracts, etc., and ensure appropriate management is implemented.

5. Voluntariness of Personal Information

Providing personal information is voluntary, but if you do not provide it, you may not be able to use the products and services offered by our company.

7 Procedures for Disclosure, etc. of Personal Information

Regarding requests for notification of purpose of use, disclosure, correction, addition, deletion, suspension of use, erasure, or suspension of provision to third parties of personal data held by our company, please contact the Personal Information Handling Office below by phone or email. We will send you an application form.

When mailing your application to our company, please use a method that allows confirmation of delivery, such as registered mail. Also, please write "Personal Information Disclosure Application Enclosed" in red on the envelope.

When submitting your application, please enclose documents that can verify your identity or the identity of your representative.

(1) How to request disclosure, etc., by the individual

If the individual wishes to disclose personal information, please fill out and stamp the form in section A below, and send it along with the identification documents in section B and section C as needed.

      Copy of driver's license, copy of health insurance card, copy of passport, copy of resident certificate, copy of residence card, seal registration certificate
    3. 800 yen postage stamp (for requests for disclosure or notification of the purpose of use)

(2) In the case of requests for disclosure, etc., by an agent

If you are entrusting your agent with disclosure, etc., please fill out and stamp the necessary information on Document A below, and send it together with Documents B through E, and F as needed, to make your request.

    1. Application for Disclosure of Personal Information
    2. Identification document (any one of the following)
      Copy of driver's license, copy of health insurance card, copy of passport, copy of resident certificate, copy of residence card, seal registration certificate
    3. Documents required for proxy identification (any one of the above B)
    4. Power of attorney (with the principal's seal affixed)
    5. Certificate of Seal Impression for the seal affixed to the Power of Attorney (issued within the last 3 months)
    6. 800 yen postage stamp (for requests for disclosure or notification of the purpose of use)

(3) Method of Responding to Requests for Disclosure, etc.

Regarding requests for disclosure, as a general rule, we will disclose the information by the method specified by the applicant in their application. However, if disclosure by the specified method is difficult, we will notify you to that effect and disclose the information in writing.
Regarding requests for notification of purpose of use, we will respond in a reasonable and appropriate manner. Regarding requests for correction, addition, deletion, cessation of use, erasure, or cessation of provision to third parties of personal data we hold, we will generally respond by postal mail or email.

(4) Purpose of using personal information obtained when requesting disclosure, etc.

Personal information obtained in connection with requests for disclosure, etc., will be used only to the extent necessary for the requests for disclosure, etc.
The documents you submitted will not be returned. After the response to your disclosure or other request is completed, they will be properly managed and disposed of.

(5) Cases where disclosure, etc. cannot be made

Please be aware that disclosure may not be possible in cases based on laws and regulations or in the following situations. If a decision is made not to disclose, we will notify you via email or other means, stating the reason.

    1. In cases where identity or agent verification cannot be performed (e.g., when the current address differs from the address on the resident record)
    2. If there is a risk of harming the life, body, property, or other rights and interests of oneself or a third party.
    3. If the personal information subject to the request cannot be identified from the information provided in the application
    4. If there is an omission in the prescribed submission documents

(6) Fees for Disclosure Requests and Collection Methods

800 yen per request (request for disclosure or notification of purpose of use).
Please enclose 800 yen worth of postage stamps (actual cost).
However, a separate fee may be charged for personal information held or records of provision to third parties that require a special disclosure method.
There is no fee for requests to correct, add, delete, suspend use of, erase, or stop provision to third parties.
If the fee is insufficient or not included, we will contact you to inform you of this. Please re-mail all necessary application documents and the required fee to the Personal Information Handling Office. If you do not re-mail them, your request for disclosure or other services will be considered as if it were never made.
Regardless of the reason for non-disclosure or any other reason, we cannot return postage stamps sent to us.

8 Security Management of Personal Information

Our company takes the following safety measures against the risk of leakage, loss, or damage of personal information.

(1) Organizational safety management measures

    1. We have appointed a manager responsible for handling personal information and operate our personal information protection management system with this manager at its center.
    2. We have established regulations regarding the handling of personal information for each stage, including acquisition, use, storage, provision, and deletion/disposal.
    3. We conduct periodic audits of the handling of personal information.

    1. We conduct regular training sessions for employees on the handling of personal information.
    2. We have employees sign a non-disclosure agreement when they join the company.
